Creating a Form Template (Updated!)

Overview:

In our newest version of the performance module, you now only have to create a single form template, and assign which persona (self, manager, peer, upward) should answer. Check out the steps below

 

Creating a Single Form Template:

  • Go to Performance > Form Templates > Add Form


  • Input the Review Form Name and Description (optional).



  • Click Add Section. A pop-up will appear where you should input the Section Name and Description (optional).


    • There is a third field with the question "Who should answer this question?". You can choose "Everyone" or "Specific Persona".
      • Everyone - This means that questions in this section will be answered by all personas in the cycle.
      • Specific Persona - You will choose which of the following will answer the section: self, manager, peer, upward (direct reports).
      • Note that if Everyone or Self is selected, the box "Hide Section and Results from Reviewee" is disabled. But if other personas are selected, you can hide the section and results from the reviewee.
        • "Hide Section and Results from Reviewee" means that any feedback from peers, managers, and direct reports will be kept from the reviewee.
  • Click Add question within a section you want to add questions to.


  • Input the Question Name and Description (optional), then choose the Question Type. Question Types are the following:
    • Text Input - Allows the user to input free text answer. 
    • Single Choice - Allows the user to choose a single answer from a list of options. You may add "Other" option, comment section, and require a comment.  
    • Multiple Choice - Allows the user to choose multiple answers from a list of options. You may add "Other" option, comment section, and require a comment.  
    • Rating - Allows the user to choose a numerical score. The allowed minimum scores are 0 and 1, while the allowed maximum scores are 5-10. You may add a comment section, and require a comment.


    • Note that you can also hide the results on the question level if you haven't. This only works if in the Section pop-up, you did not choose "Everyone" or "Self", and have not clicked "Hide questions and results from reviewee."
  • Click Save. Do the same for other sections you have created to add questions.
  • Once done adding the questions, click Save. The newly created template should now appear in the list of the Form Templates.
